Case Review Sub Committee
Our
aim is to undertake case reviews where a child has died or � in
certain circumstances � been seriously harmed, and abuse or neglect
are confirmed or suspected. To make sure that any lessons from the case
are understood and acted upon. See DoH, �Working Together to Safeguard

Objectives
-
To
share information and determine whether areas of concern should
result in an interagency inquiry and to set terms of reference for
each inquiry.

To
give strategic management and leadership to complex investigations
and to ensure liaison between agencies.

To
commission, where necessary, comprehensive investigations of cases
and to ensure that the recommendations, progress and outcomes
arising out of a serious case review are actively monitored
-
To
ensure that appropriate personnel are engaged to conduct the
investigation given the nature and sensitivity of any investigation.

To
assess and manage the risk of impact of the investigation and the
potential outcomes for the individuals, communities and agencies
concerned.

To
report to the GSCB the findings of the investigation, with
recommendations for action as appropriate.

Executive Summaries are used to disseminate lessons learned in the
Serious Case Reviews that are undertaken by the GSCB SCR Sub-committee. They are
available to the general public and professionals through the GSCB
website. To view these documents, please select the links

Summary of a Serious Case Review is a public document and therefore the
names of the child(ren) and families, are disguised to preserve
anonymity.
